How Much Does a GLP-1 Cost? The Real Monthly Numbers

The number in the headline is almost never the number you pay. Two women can fill the same prescription in the same week and be hundreds of dollars apart.

That is not a scam. It is how drug pricing works here, and once you can see the pieces, you can plan around them.

So: what the manufacturers publish, what insurance does to it, what HSA and FSA money can cover, and where coaching sits in the same budget.

What drives the wide range in what people pay?

Six things move the price, and only two of them are inside your control.

The biggest is whether your plan covers the drug at all. After that: which brand, which dose, whether you qualify for the manufacturer’s savings program, where you fill it, and whether the product is FDA approved or compounded.

You are never comparing one price to another price. You are comparing your plan, your dose, and your pharmacy to somebody else’s.

That last row is the one I would not shortcut. The FDA has documented dosing errors and hundreds of adverse event reports tied to compounded semaglutide and tirzepatide, including patients dosed beyond the approved labels. Money saved there is not really saved.

How does insurance, or its absence, change the monthly cost?

With commercial insurance that covers the drug, the savings cards are the cheapest path. Novo Nordisk’s Wegovy offer says covered patients can pay as little as $25 a month, with the offer applying up to $100 of the cost each month. Lilly’s Zepbound savings card says as little as $25 for a one, two, or three month fill, capped at $1,300 of savings a year.

With commercial insurance that does not cover it, Lilly publishes a KwikPen price as low as $299 for a one-month prescription.

With no insurance, you are looking at the self-pay numbers. Zepbound is listed at $299 for 2.5 mg, $399 for 5 mg, and $449 for 7.5 mg and higher. The Wegovy pen is listed at $349 a month for the 0.25 to 2.4 mg doses and $399 a month for the 7.2 mg strength, with a new-patient offer of $199 a month running through December 31, 2026.

On Medicare, both manufacturers list a bridge program at no more than $50 a month for eligible Part D patients.

Every one of those numbers has conditions attached, and every one can change. Check the manufacturer’s page and your own pharmacy before you build a budget on any figure you read anywhere, including this one.

Ozempic and Mounjaro are priced separately by the same two manufacturers, and there is no figure for either here. Look them up directly rather than assuming they track the weight-management brands.

Can HSA or FSA funds be used for the medication, the coaching, or both?

Sometimes, and the rule is narrower than most people expect.

IRS Publication 502 says you can include amounts you pay for prescribed medicines and drugs as medical expenses. It also says you can include amounts you pay to lose weight, but only when it is treatment for a specific disease diagnosed by a physician, such as obesity, hypertension, or heart disease. And it says plainly that you cannot include membership dues in a gym, health club, or spa.

So a prescribed GLP-1 generally sits on the eligible side. Nutrition and fitness support sits in the conditional column, and it turns on a clinician’s diagnosis and documentation, not on how badly you want it to qualify.

How does adding coaching change the total monthly budget?

The medication changes your appetite. It does not build the habits, the protein intake, or the muscle you want to still have in five years.

That part is what coaching is for, and it is a separate line on the budget.

Coaching runs on a six month minimum or a discounted twelve month term. After the commitment is fulfilled you can cancel with thirty days notice, and it rolls to month to month.

If the App at $49 a month is where your budget lands this year, that is a real starting point, not a consolation prize. Start where you can actually stay.

What financing exists if cost is a barrier?

What is not on that list is a cheaper unapproved source. That is the one place where the savings are not savings.

What can coaching actually do for the money, and what can it not?

Coaching is not medical care. Peptide and metabolic support, including GLP-1 options, is offered through a licensed medical team. It is never required, and it is not where anyone starts.

What should you actually budget for, month one?

Write down four numbers before you commit to anything.

One, the medication at your likely maintenance dose, not the starter dose. Two, the visit or membership fee for whoever prescribes. Three, the support that keeps you from losing muscle and undereating. Four, what happens to lines one and two if you stop the medication.

Most people budget for month one and get surprised by month seven. Plan for the maintenance dose and the exit, and the whole thing gets calmer.
