The medication quiets your appetite. It does not tell you what to eat, how to keep your muscle, or what happens the month you stop. That part is coaching, and it is what I do.
HSA and FSA eligibility is available through Truemed for Julie Salter Fitness. Check your own plan before you count on it.

GLP-1 coaching is the nutrition, strength, and habit plan built around the medication, not the prescription itself. At Julie Salter Fitness that means custom macros with a real protein target, strength training that protects your muscle while the scale drops, weekly accountability, and a plan for after medication. Medical support, if it fits you, comes through a licensed medical team.
Because the prescription handles appetite and nothing else. Both the Wegovy label and the Zepbound label say the drug is indicated "in combination with a reduced calorie diet and increased physical activity." The label assumes someone is building that part. Usually no one is.
Muscle. In the STEP 1 body-composition substudy (Journal of the Endocrine Society, 2021), 140 participants were scanned with DXA. Over 68 weeks the semaglutide group lost 15.0 percent of body weight, and total lean body mass fell 9.7 percent from baseline. Lean mass is not only muscle, and the trial did not test what strength training would have changed, but the direction is clear: some of what leaves is not fat.
Protein. When you are not hungry, protein is the first thing that quietly disappears from the plate. Nobody feels a low-protein day. You feel it months later, in your strength and in how the weight comes back.
Habits. A quieter appetite is a window, not a skill. If the eating patterns underneath do not change while the window is open, nothing is different when it closes.
The after. Most programs treat the medication as the finish line. Here, the plan for after starts on day one.
"Anyone can hand you macros. This goes four layers deeper."
Handing a woman on a GLP-1 a macro target and calling it coaching is not coaching. Coaching is what happens between the injections.

The 4B Approach is how I coach everyone. On medication, each layer changes shape. The full method lives on its own page: how the 4B Approach supports you on GLP-1 medication.
What the body is actually doing. On a GLP-1, appetite is quieter, so two things matter more, not less: protein and strength.
The ISSN position stand on protein and exercise puts daily intake for most exercising adults at 1.4 to 2.0 grams per kilogram of body weight, spread across the day. The HHS Physical Activity Guidelines say adults need muscle-strengthening activity at least two days each week.
You cannot out-discipline a system working against you, and on a GLP-1 that system now includes an appetite that will not remind you to eat.
The skills, so you stop guessing. The new skill on medication is eating enough when you are not hungry: protein first, meals on a schedule instead of on a feeling, food you can actually eat, on a Tuesday, in your real life.
Training that builds you instead of draining you, because you are recovering on fewer calories than before.
Who you are on the bad days. Not mindset. Mindset is the story you tell when things go well. Belief is what you do when they do not.
On a GLP-1 the bad day is the week the scale moves fast and you start to believe the medication is doing everything, so you stop doing your part. You can't hate your way into a body you love, and this is the layer where that finally ends.
Making it hold in the life you actually have. The busy season, the sick kid, the travel, the grief. Injection days, nausea weeks, dinners out where you eat four bites.
Most plans are built for a life with nothing else in it. Yours gets built for the one you are living.
"On a GLP-1, the quiet appetite is the easy part. Keeping your muscle and your skills is the work."
Julie Salter Fitness does not prescribe anything. Here is the whole process.
Start at juliesalterfitness.com/assessment. It asks about your history, your goals, and whether medication is even on the table for you.
They review whether you qualify and prescribe only if appropriate. Julie Salter Fitness does not prescribe, choose a dose, or change one. Not everyone is a candidate, and that is a fine outcome.
Custom macros with a protein target, a strength program you can recover from, and weekly check-ins. This is the part I run.
Not in month six. The eating skills, the strength base, and the habits are built while the appetite is quiet so they are already there when it is not.
"This is never required, and not where we start. But for the right person, it can be a game changer, offered as a tool, never a shortcut."
Peptide and metabolic support is offered through a licensed medical team, it is never required, and it is not where anyone starts. If you want to know who is behind each part of this before you begin, the team page spells out the coaching role and the medical role separately.
The line I hold. The medical team decides whether medication fits you. I decide nothing medical, and I would not want to.
The honest answer comes from the trial data, not from me. In the STEP 1 trial extension (Wilding and colleagues, Diabetes, Obesity and Metabolism, 2022), participants stopped semaglutide at week 68 and were followed off treatment for another year. Those who had been on semaglutide regained 11.6 percentage points of the weight they had lost by week 120. The authors summarized it as roughly two-thirds of the prior weight loss regained one year after stopping, with most cardiometabolic improvements moving back toward baseline.
Those are trial findings in a group that received a lifestyle program alongside the drug and then stopped both. They are not a prediction for you. But they are the reason I refuse to build a plan that only works while the prescription is active.
So the strength base is built during the medication, when it is hardest, so it exists after. The protein habit is trained until it is automatic, not until the scale says a number. And the food skills are practiced in real life (wine nights, holidays, dinners out) while the appetite is quiet, because that is exactly when the old patterns come back.
I lost 70 pounds and I've kept it off for over 4 years. I am not against medication. I am against plans with no after.
"My goal isn't just weight loss. It's helping you keep it off, and feel confident doing it."
That decision belongs to a clinician, not a coach, but both labels are public. The Wegovy label says it is contraindicated for anyone with a personal or family history of medullary thyroid carcinoma or with Multiple Endocrine Neoplasia syndrome type 2, and for anyone with a prior serious hypersensitivity reaction to semaglutide. The Zepbound label lists the same thyroid history and known serious hypersensitivity to tirzepatide.
Both labels carry warnings about pancreatitis, gallbladder disease, kidney injury from dehydration, and severe gastrointestinal reactions. The licensed medical team reviews all of this before anything is prescribed.
"Coaching cannot tell you whether a medication is safe for you. It can make sure you are not on one without a plan."
Medical support is a piece you can add, not a separate program.
| Option | What it is | Medical weight-loss tools | Price |
|---|---|---|---|
| Build Your Own Program | You pick the pieces, starting from JSF Vault, Group, Most Popular, or All In. The total updates as you choose. | Can be added as a piece | The price moves with you. See the configurator. |
| Foundation Coaching | Custom macros, custom workouts, and check-ins | Can be added separately | $500 a month on six months, or $475 a month on a twelve month term |
| Elite Coaching | Weekly 1:1 Zoom calls, custom macros and workouts, weekly diet audits | Included | $1,500 a month on six months, or $1,450 a month on a twelve month term |
Foundation and Elite run on a six month minimum or a discounted twelve month term. Affirm and Klarna are available on both. After the commitment, cancel with thirty days notice and it rolls month to month.
HSA and FSA eligibility is available through Truemed for Julie Salter Fitness as a general site fact; your plan and Truemed decide what qualifies, not me. Medication costs are separate and are set by the pharmacy and the medical team, not by Julie Salter Fitness.

No. Peptide and metabolic support is offered through a licensed medical team, it is never required, and it is not where anyone starts. The 4B Approach is the program; medication is one optional tool inside it.
No. Julie Salter Fitness does not prescribe, choose a dose, change a dose, or direct tapering. If medical support could fit, a licensed medical team evaluates eligibility and prescribes only if appropriate. Coaching builds the plan around whatever they decide.
Yes. Coaching layers on top of care you already have. Your prescriber keeps every medical decision. I build the protein target, the strength program, the check-ins, and the plan for after, and I will never tell you to change your medication.
I set that number individually in coaching. As general education, the ISSN position stand puts daily protein for most exercising adults at 1.4 to 2.0 grams per kilogram of body weight. The free protein calculator gives a starting range, not a personal target.
Some lean mass loss showed up in the STEP 1 body-composition substudy: total lean body mass fell 9.7 percent while body weight fell 15.0 percent over 68 weeks. That is why strength training at least two days a week and a real protein target are in the plan from week one. The full plan is in the semaglutide muscle loss article.
HSA and FSA eligibility is available through Truemed for Julie Salter Fitness. Whether your plan and your purchase qualify is decided by Truemed and your plan administrator, not by me, so check there first.
Take the assessment. It tells us whether medication is even a fit, and either way you leave with a real plan. Take your time. There's zero pressure or rushed sales tactics.
